In most cases our physicians will require a referral from your primary care or other specialist in order for you to be seen.
Please arrive 15 minutes prior to your appointment if you have already completed your paperwork. If you are unable to complete your paperwork online or print it out, please arrive 30 minutes prior to your appointment to fill out paperwork in our office. Due to our implementation of electronic health records if we do not have your information entered in to our system before your appointment we may have to reschedule you for a later date and time.
Most new consultations require approximately 40 to 60 minutes of time. Please ensure that any relevant medical records, including MRIs, CTs and X-Rays be collected before seeing us. In some cases our physicians will be able to look these reports up online. Please be sure to bring a current list of medications with you.
Plans for ongoing care can vary by patient and by problem. Please be aware of the increasing demands being made for our services and the fact that we will need to act as consultants for you and your primary care provider in many instances.
Follow-up appointments generally require 15 to 30 minutes. We strive to be on time for all scheduled appointments. If you are not checked in at our office fifteen minutes prior to your appointment we may have to reschedule for a later date. We respectfully request that you provide 24 hours notice for canceled appointments. Appointment reminders are given by phone 48 hours prior to your appointment. A charge may be assessed for missed appointments.
